False Emergency Alert in Hawaii
These are remarks given by an attorney adviser of the Federal Communications Commission to its open meeting on 30 January 2018, setting out the Public Safety and Homeland Security Bureau's early findings on the false ballistic missile alert sent in Hawaii on 13 January 2018. They record that the alert went out at 8.07 am and was corrected at 8.45, that no procedure prevented a single person from sending it, and that the software asked the same confirmation question for a test and a live alert. The officer who sent it said they heard that this was not a drill but did not hear that it was an exercise, which the Bureau could not fully assess because it had not been able to interview the officer. The remarks also record that the agency had adopted a policy requiring two credentialed warning officers to validate every alert and test. This page cites them for the facts of the alert and the safeguard.
